MOBILITY IMPAIRED (handicap) HANG TAG

Per the Louisiana State Legislature RS 47:463.4 under section D. The secretary shall not issue special license plates, hang tags, or mobility impairment identification cards except as designated in this Section or in R.S. 47:490.4. Any person with a mobility impairment whose impairment is permanent may obtain a hang tag or mobility impaired identification card at no additional fee other than the issuance cost of seven dollars and fifty cents. Notwithstanding any other provision of law to the contrary, except as provided in Subsection C of this Section, the secretary shall not charge any fee in excess of ten dollars for the issuance of special license plates for persons with mobility impairments.


E.(1) The term "person with a mobility impairment" shall include any person who is impaired because of any of the following conditions:

      (a) Cannot walk two hundred feet without stopping to rest.

      (b) Cannot walk without the assistance of another person, walker, cane, crutches, braces, prosthetic device, or wheelchair.

      (c) Is restricted by a lung disease to such an extent that the person's forced (respiratory) expiratory volume for one second, when measured by spirometry, is less than one liter, or the arterial oxygen tension is less than sixty mm/hg on room air at rest.

      (d) Uses portable oxygen.

      (e) Has a cardiac condition to the extent that the person's functional limitations are classified in severity as Class III or Class IV according to standards set by the American Heart Association.

      (f) Has a diagnosed disease or disorder, including a severe arthritic, neurological, or orthopedic impairment, which creates a severe mobility limitation.


1.  Medical certification

  • A Physician's Certification of Mobility Impairment form (for handicap placards and license plates), DPSMV 1966, which has been completed by the mobility impaired individual's physician.

2.  If the mobility impaired person has a Louisiana ID card/driver's license with a clearly identifying photo and their Social Security Number is on file, additional identification is not required. Otherwise, the applicant must provide the same identification documentation as when obtaining an ID card/driver's license.

The expiration of the hang tag will be determined by the impairment status listed on the certification. 


Individuals with permanent impairments will receive a four-year hang tag.  Recertification is not required upon renewal. 


Individuals with temporary impairments will receive a hang tag that is valid for one year from the date of issuance. Recertification is required to renew the hang tag. 

Only one renewal is allowed for a temporary impairment.

 Those with “Permanent Impairment” or a Veteran who has a 50% or more “Service Connected Disability” are eligible for the handicapped plate. All documents to issue the plate are requrired.

Military veterans with a disability rating of 50% or greater have the option to receive a Military Honors plate.  Plates are in very limited supply. Please call and verify availability.
